Default hard grind when going into 5th..

I have a hard grinding sound only when I go into 5th gear. Can anyone help?

If it's 5th gear (and 5th gear ONLY)

Honda had a TSB on this condition. Related to the trans being (or had been ) refilled with the incorrect trans fluid. This effectively starved lubrication in portions of the tranny case; effectively trashing 5th (and sometimes 4th as well)
Try a Drain/Refill using the correct Honda lube and see what happens.

If it does not help, you're most likely looking at a teardown involving Syncro's and Shift Forks.

I friend of mine had his 5th gear grinding on his 94 accord, he drained the tranny fluid, and refilled with synthetic oil, and amazingly, it had "solved" the grinding. Now that's just one opinion before you go ahead and take your tranny out and put another one in.

You could also give syncromesh trans fluid a try. A lot of guys swear by it. It's a GM product, so you can get it at any parts counter, but it's also available at some auto parts stores

Honda has a new and improved MTF that beats synchromesh IMO. It solved the grind my 89 LX-i had.
