Default 2 pressure plates in 3 months why?

hey guys..i have a b16a1 jdm spec with a y1 tranny non lsd..
and the pressure plate keeps on messing up... im not driving bad or droping the clutch hard...

i broke the clutch in easily for 1000 kms... and it didnt mattter it still seemed to let go... all the pressure plate teeth? are bent and the clutch disc is burt completly on only one side... any reason?

bad trans? i replaced the pilot and the relese bearings aswell ...
could my fidenza flywheel be the problem?

what clutch cable are you using? Maybe its too tight and its always engaging the clutch
