It's your lucky day. I took these a while ago for a previous requestee.

Nighthawk GS-R has a set for sale for a really good price. They retail for $220.

The USDM switch is $20.

The JDM switch sits in place of the cruise control on/off button and costs closer to $60.

IN VTEC: thanks for the pics! very helpful. do u need to go in with a dremel and cut out the plastic piece or does it pop right out?

UltraGSR: thanks!

also, i saw someone selling OEM Raybrigs or something like that. is that who makes the USDM fogs? how about JDM?

can the usdm fog light switch be used for the jdm foglights, because i like my cruise control. I'm planning on doing my conversion pretty soon and i've been wondering about the foglights for the longest time.

It pops right out from the back. Some people even use the spare piece to dremel it out to make a manual antenna switch so that the antenna doesn't pop up every time the radio is on.

the OEM foglights should be made by Raybrig. i had some on my 98 civic. they were $220 w/ toggle switch.
got them from http://www.hparts.com
but i think they should be exactly same for tegs, the only difference being the toggle switch...
