Bad Idle in gear!!! Help please!

Default Bad Idle in gear!!! Help please!
My 98 Civic hx with 80k miles is idling horribly while in gear and warmed up! The car is completely stock. Please any suggestions? I have done all necessary maintenance and cant figure out whats causing it to fluctuate so bad at idle!
