1993 Accord EX starting problems & staying started

im just about to an end here with my 1993 accord EX....
When I start it sometimes its fine, jumps to the cold idle at about 2k rpm's - idles fine. until it gets warm, then goes to normal idle.
other times, it jumps to the cold idle when starting (as described above) then promptly stutters then dies, or when i hit the gas.
and other times it just won't start at all, or starts really rough and then dies right away.
Does not matter if its cold out, damp out or warm... been having this problem since September, but now I have to depend on this car and gotta get this fixed!!! GRRR!!!
I've replaced plugs, wires, cap, rotor, igniter and the main relay up underneath the dash on the drivers side. The only code I get when i do the jumper trick is "41" - which is an O2 sensor fault, which I don't think would cause the symptoms as described.
anyone please lead me into the right direction!!

The 02 sensor tells the ecu what to set the fuel mixture at,so it could be that.Also a dirty iacv and fitv can cause hard start problems.Do a search for iacv and fitv as there is a good write up on it.Mark
yeah, hopefully i did. So far, the problem has not happened again. All i did was clean out the IACV and FITV - which a good link can be found here https://honda-tech.com/zerothread?id=2153910
I took each off and throughly cleaned out with brake cleaner and flushed that screen on the IACV....
like I said, so far so good.
