Default Getting Code 67 in '98 DX. What else do I need to fix?

Ok I pulled the code its a 67, that is the only one and i know its the Cat Converter code.

The car is a LEV (low emissions vehicle) 98 DX Auto

What would cause it to throw the code? Do I need to replace the Cat? Would a bad o2 also cause this but not throw a code?

The car has a leaking exhaust manifold gasket, that i've been needing to replace. Would that cause it to throw the code because its not seeing the correct amount of exhaust through the 02?

Thanks

Replace that exhaust gasket. My wife's LX (same engine as your DX) threw that code constantly because of a fairly sizable crack in the exhaust manifold. Fixing that cleared it up.
