Default Starts and Dies when Cold

Ok, a buddy of mine has been having this problem for a rather long time, so i decided I'd be a good friend and sit down and trouble shoot it a bit for him but i've somewhat hit a wall. The car is a crx, with a b16 swap, msd ignition, ect.ect.. I name the ignition system simply because it seems to be an ignition problem. Now what happens is, when the car is cold it will fire up, run for about 5 seconds and die and then throws a code 15 which is the ignition output signal. I've trouble shooted the code, and everything tests perfectly fine. I've recently just swapped ignition modules(ignitor) with a known good one and the problem still persists. The kicker of it all is, once the car starts to warm up, it run's and starts fine. Any help or insight someone can give on this problem would be greatly appreciated, because i'm somewhat tired of having the car in the shop, and need to get it out and can't really afford the time i've already put into it as a favor. Swap the coil too. Sometimes the coil can kill an ignitor or vise versa.
