92 always stuck in D4, no matter what gear you are in
It's got 211k on it, tranny was redone at 115ish. The D4 light is always on. Other gears work fine even though two gears are lit up. When in drive it hardly goes, reverse and drive 1-3 work fine. Is the tranny shot or is something just stuck?

This sounds like a TCU problem. Does the D4 lite stay on when you turn the key on?
You can jumper the BLUE 2 pin connector above the passenger side kick panel and get the 'flash' code when you turn the key on.
