I am in the middle of a head gasket swap on my girlfriend's 93 GS (B18a)

I have the head off, and a new gasket ready to go on.

a few questions-

1) What do the head bolts torque to? inside to out, 2 passes?
2) What do the cam caps torque to? inside to out?
2.5) Anything special needed to reinstall the cams?
3) Any problem with a felpro gasket?
4) Any suggestions for cleaning the head/block mating surface? It looks a lot cleaner than the organic gaskets I am used to.

Bonus question -

Every once in a while, her car wont start for a few minutes. Starter turns over fine, but the car wont fire. Always after a few minutes it will. Is that a common problem?
