This is the PPC-67000 from Audiovox. It was used on the SPRINT PCS network:

The phone comes with (1) stylus, usb cradle and charger,carrying case/pouch, and original box (came with phone). Unfortunately, I lost the CD that came with it, but you can download the software online.
