Default tilton clutch... no more splines on the disc

i got a tilton twin disk clutch that have around 2k miles on the clutch and as i was driving the other day i was hardly able to get in gear and when i step on the clutch almost all the way i hear a grinding noise from the tranny or the clutch. I dropped my tranny and found one of my clutch disk (closest to the pressure plate) was rounded off. I stick my clutch alignment tool inside the disk and it would just spin freely. I've remember it happening to someone else on the forum and i searched and found that it happened to jordo@KAP also. Did this happen to anyone else or is it just us? Now i need a replacement disc, anyone know where i can get one? And what can i do to prevent it from happening again? I wasn't using tilton's hydro release bearing also.

yeah i will contact them... but im not really expecting anything from them... we'll see how it goes
