Whats Involved. Converting 4 Lug to 5 Lug?

Just as it says. What parts are needed in Converting a 95 Teg LS, 4 Lug to 5 Lug? And were can I get these parts.

This is a pretty helpful place

Just look for an ITR suspension/brakes swap, they're usually on ebay a lot.
