Looks like laguna seca, how was this driver thrown from his vehicle if he had a proper restraint system (i assume it wasn't).

was there something they overlooked during construction of the car? i assume with the right crash this sort of this can happen to my civic (or any car)? should i beef up the seat mounts?

im not sure if you have ever been to a SCCA/NASA race day but some of the ways people have there seats mounted is down right crazy.
Also he could have just not had his harness on tight, a harness will strech up to 3 inchs, so you need to keep that thing SINCHED down on you.

I think alot of car builders sometimes assume that the distances between certain items in a race car will remain constant, and count on that fact. That biggest being the seat back and whereever you have your shoulder straps mounted. All it takes is the cage or floor of the car to bend a few inches to give the slack in belts you need to get ejected. In this case the cage deformed enough to both loosen the belts and allow the window net to open.
Obviously making sure to mount belts and seats properly(HUGE backing plates) is key, but also, use seat back brace of some kind even if your seat is FIA. When you design you restraints system(this means window nets too), try to anticipate what you are mounting to changing shape and location, at least a few inches. Because it is more than likely that the moment you are counting on this equipment, it won't be the right shape.

Man... That kinda makes me sick. Do you think a window net would've stopped him from ejecting like that? Or maybe he already had one...?
It seems like his seatbelts made everything worse by holding him partially outside the window as the car tumbled.
