Default WTF Crank pulley fell off!?

My crank pulley is about to fall off!

I was driving and i notice a weird noise. Then my battery light comes on. I stop to check what it is and my alt belt fell off. OK so no big deal. I look into it a little more and i notice that the crank pulley is loose and just kinda free wheeling. How the hell can a crank pulley just get loose like that? I hope the key way isnt jacked up. Is this a common problem? Or is a douche bag mechanic the culprit? I just bought the car but im sick of these small problems popping up.

I was at the strip once and on my third pass, I felt more vibrations right when I shifted into 4th. As I drove my car back, the vibrations didn't feel normal and my battery light came on. The crank pulley bolt came loose and the pulley was crooked. It shredded my alternator belt. The key way was fine. I just remove the pulley and straightened everything and bolted everything back.

I don't know how this happened cuz I ran plenty of times down the track on numerous occasions.

Maybe someone didn't tighten it when you work on it last? I don't remember the exact lb-ft the crank pully bolt is supposed to be, but it is pretty high.
