HELP! PLEASE!

My car has been acting like crap for a few months. What has seemed to happen is the car when it first starts will idle around 1k-1.5k rpm's then when it warms up it seems to fluctuate in idle. the idle go's up and down from 200-800 up and down. then sometimes it just drops to zero and putters. Saturday it started up cold just puttering and it was smoking (white smoke) and could smell like it was running hella rich(reminded me of a motorcycle needing to adjust the carb.) anyhow I've changed my spark plugs, wires, dizzy cap looks good only a year old, fuel filter,cleaned throttle body, ran injector cleaner through and have just been puzzled. yesterday I changed the oil and plugs again this time w/ platinum's and tossed the coppers. thought it may help took a long drive to see if I could get it to act up and on the highway around 80 in 5th gear (thats roughly 4500 rpms) the car would bog down then regain power and the whole way I kept hearing these little back fires from time to time. this is killing me I don't have the money to take it to honda. today on the way to work I got code 45 so finally I've got something to work with but this is the first code I through. (BTW, I replaced the primary 02 within the last year and the cat)

I know my system must me too rich thats why I got the backfires rite? I don't think my exhaust is leaking. I did double check yesterday..
