Input Shaft Bearing

Input Shaft Bearing
My tranny has all the signs of the input shaft bearing being shot
and I found this

which can get me almost to the right spot..I am just not exactly sure what part number I need
Any help would rock guys.

017 91004-P21-003 BEARING (28X72X18) 1 1998 Acura INTEGRA 58.97 49.14
016 91002-PS1-003 BEARING (28X60X15) 1 1998 Acura INTEGRA 34.95 29.12
and you may as well grab the seal there also..
021 91216-PH8-005 OIL SEAL (28X41X7) 1 1998 Acura INTEGRA 7.88 6.57
017 91004-P21-003 BEARING (28X72X18) 1 1998 Acura INTEGRA 58.97 49.14
016 91002-PS1-003 BEARING (28X60X15) 1 1998 Acura INTEGRA 34.95 29.12
and you may as well grab the seal there also..
