Default about fogged headlights

when headlights get fogged or turn yellowish...is this all from the outside even if it looks like it could be the inside?? or what percent of it is from the outside and the inside??

well i've heard of people fixing this by polishing out the plastic, using some highgrit sandpaper and then plastic polish.

so if thats possible i'd assume it's outside only?
