I have a 92 Accord Automatic, do they have torque converters? At 70 mph when I push the gas the motor revs about 300 to 400 rpm then settles down.

its an automatic.. of course it has a torque converter

 

it's releasing the lockup and this is normal when you depress the gas petal down beyond a certain point..

Normal Condition if I understand what you're talking about. That's the Lock-up Converter disengaging (400rpm jump) and then reapplying.
The harsh shift may be a problem with one of the shift solenoids (#5, #8 in illustration)
