There are several reasons which can cause a transmission to grind. And you have touched on one of them. They are:

A) Grinding in ALL Ranges
1) Incorrect Clutch Adjustment
2) Incorrect Lubrication Fluid (Too Thick Fluid)
3) Worn / Damaged Clutch Pressure Plate (More previlent on Non-OEM Replacement units)

B) Grinding in Single Gear Ranges
1) Worn Syncro Ring / Shift Fork
2) Worn Gear from Damaged Syncro's
