1998 Accord Post Timing belt replacement troubles

Its my sisters 1998 Accord with the F23a1 engine. It just reached 90k and we decided it needed a new timing belt.
Well I started on Sat night and got stuck in on Sunday. Everything looked right.. 4 hours later its back together. It runs fine but vibrates. I suspect the counterbalance belt.
I pull the entire car apart again and check it. It looks 1 tooth off. So I put it all back together and its better but not great.
Then tonight I decide, this isnt honda quality and I pull the entire thing apart again and it looks 1 tooth off again. It still vibrates at high rpm.
I have a repair manual and I did make sure everything was set off of TDC and inspected that all the marks lined up with the assigned arrows.
WHAT ON EARTH am I missing ?
I am ready to pilot the damn thing into a tree!

It has got to be the balance shaft belt. I'm having the same
problem with mine right now, and i cannot figure out what it
is. The one thing i noticed in the book, was that it stated not
to use any air tools to put back the pulley bolt. I am not sure
if that is the problem or not because i am doing everything
else the same exact way the book puts it. (Align front balance
shaft with the mark on the block, and insert the 6*100 allen
wrench through service hole in back of the block).I've torn the
car apart about 8 times, if you figure anything out let me know.

Anways, how about the motor mounts? If they are worn out, they can cause vibration.
