Water leaking from rear and front AC vents (2003 Accord EX). What could be causing this?
The rear carpet is wet and I'm assuming that something is not draining right with the AC system? Any suggesions on correcting this?

One of the byproducts of a really efficient a/c system is water condensing on the cold a/c register. (much like the water on the outside of a cold bottle of beer).
The only place in a car where a malfunction can actually cause a problem is just in front of the glovebox where the a/c evaporator core and ductwork is located. Just like the a/c outlets, the evaporator can get so cold that large quantities of water form in the evaporator case. This normally drains out a tube (usuall visible on the outside right side of the firewall). If this tube should clog from mold or other, the water will not escape as designed and start leaking into the vehicle.
You can minimize the formation of water by condensation by operating the a/c in the "recirculation" mode. This way the a/c recools air which has already been cooled before, and you don't draw into the car outside air laden with moisture
