I've got a accord ex w/158k miles.

problem: when driving at a steady speed; 35,40, 45, 50 doesn't matter.
the gas pedal jerks and i feel a hesitation/shaking from the gas pedal and car, but not the steering wheel. this happens worst when driving in 5th gear, not as bad on 4th

if i press on the gas pedal to increase the speed; it runs great with no hesitation.
hisitation/jerking/ only when trying to drive a a contstant speed.

starts fine and idles fine when at a stop light.

air filter and ecv filter have been replaced. tires rotated and balanced.

any ideas?
