Default code 41 after intake install?

i got a code 41 (primary oxygen sensor heater) on my buddy's stock 95 civic ex. i tried to reset it several times, and its been like 4 months and the CEL stays on. do i just need to replace the o2 sensor? why would it just go bad at the same time as the intake install? and which sensor is it?.. i did a search but couldnt find out exactly what a primary oxygen sensor HEATER was.

it seems like just a coincidence. the primary o2 sensor heater just heats up the element so the ecu can run in closed loop mode sooner. to replace the heater you must replace the entire o2 sensor.
