Default '91 Integra LS exhaust

Will the exhaust manifold on a Honda B16A motor line up right with the exhaust system on a 1991 Integra LS?

It should, I believe the rule is b-series to b-series. Hopefully some more knowledgable people will get in here to confirm that though.

-if you mean will the downpipe bolt up the exhaust the answer is NO.
-Will the upper part bolt to the head the answer Yes.

In order to avoid any modifications just buy a header thats intended for a 90-91 integra rs,gs,ls doesn't matter but the downpipe will be lower to the ground and may cause problems if your car is lowered. I have a b16 in my 91' and I have problems going over bumps with it lowered

it should bolt on, but the issue is oil-pan clearance i don't think it will work

Hm... what about a 1990 w/ a jdm b20 from a 98 crv? The DC Header is cracked so I was thinking of getting something...

i have a b16 header on my 91 and it bolts up to the head but the down pipe slants toward the right so i had to modify my exhaust

Yes the upper part of the header will bolt up but you'll need to extend or shorten your exhaust to bolt up the the downpipe.
