What's the firing order, when looking at the engine, of the spark plugs? And what are the orders of the wires from the distributor?

Also, why does the end spark plug (driverside) pop out and make a slapping noise when the car is running?

This sounds weird but the threads on the spark plug might be stripped (rare but it happens). It should say on your distributor what order they go it. From left to right it starts with one.
