it's usually done in Photoshop. Select the wheel, and put on a radial blur.

but im assuming you take a picture of the car moving somewhat because you can see that the body is clear but the reflections on the body have a motion blur to them like the background does

your correct, the car is actually moving.
most people wont tell you how they do it exactly because its their business, how they make their money in most cases.
but in general, its a tripod connected to the outside of the car. ill try and find you some pixors of what it looks like
