Default Air conditioning in 1991 Honda Civic DX

The AC unit on this one owner car with 196,000 miles stopped working last fall and I suspect the electric clutch is the problem. It was a Honda dealer installed unit. The AC now functions intermittently. I have several questions:
1) Can you suggest a test(s) that will reveal whether the problem is the clutch?
2) If the clutch has failed, should I replace the field coil as well as the pulley, bearing and hub. Those two units are sold separately.
3) Would it be wiser to install a completely rebuilt compressor? If so which re-builder has a good reputation for quality?
4) The AC is still filled with R-12. Can the clutch be replaced without removing the R-12 and refilling it with R-134?
5) Can you recommend a reliable R-12 to R 134 refit kit
