how to adjust timing belt tensioner while motor is in the car?

Default how to adjust timing belt tensioner while motor is in the car?

i know it's easier to loosen/tighten the belt tensioner while the motor is out of the car, but is there a way i can do it WHILE the motor is still in my car? there's hardly any room to work in there. i have a b16 in a 93 civic 4dr and having really bad throttle response. thank you.
