Tire balancing?
I always get lifetime warranties when I buy tires, which includes balancing and rotation for free, but thats entirely up to you. As for price, call around and find out, all shops charge differently.
Yeah, I also pick up the lifetime rotation / balance warranty when I buy tires (~ $8 per tire). The money is paid back within 2 rotations (15,000 miles).
Call your local Wal-Mart and ask how much they charge, then call other places keeping the Wal-Mart price in mind.
As for whether or not you should get a lifetime warranty, it really depends on how long you plan to keep the car. If you plan on selling it in a year or two, then no, don't get the lifetime warranty. If you plan on keeping it for awhile, then yes, pick it up, because you'll (hopefully) be rotating your tires every 7,500 miles.

Yep, I get mine rotated and balanced every other oil change.
