The only unsolved issue for me at the moment is the brake lines are too short from the caliper to the mounting bracket on the knuckle so I'll have to figure out a way to make that work or buy aftermarket lines.
I purchased all the parts new, part numbers are here:
QTY 2: Front Premium Rotor by BREMBO - 09.9936.11
QTY 1: CARDONE INDUSTRIES - 19B1460 - Front Right Rebuilt Caliper With Hardware
QTY 1: CARDONE INDUSTRIES - 19B1461 - Front Left Rebuilt Caliper With Hardware
Here we are nearly 20 years later.
Im attempting to do the same on my 91 CRX Si as Im glazing my Bosch semi metallics during track days.
I ordered the same calipers from Cardone but received one 23T bracket and one 25T bracket. So I ordered two more calipers and got another 23T and 25T pair.
Within this thread there is some speculation that the 23T bracket will work for the 262mm 91 Civic EX rotor.
Max Diameter to 17CL15VN caliper interference:
23T - 257mm
25T - 264mm
Per my measurements no, the 23T bracket will not work.
I can also confirm both of my 25T brackets needed a small amount of grinding to clear the knuckle.
Similar to others earlier in this thread my brake pads required a small amount of the backing plate ground away on the outside pad.
My aftermarket "Techna-Fit" stainless steel brake hoses fit without modification.
I am leaning towards chopping up NB Miata rotors to turn into a spacer. I happen to have a scrap set.
NB Miata rotors are also 4 x 100. They are 6.3mm thickness at the top hat. OD fits within the ID of the EX rotor top hat.
This could mean minimal machining required.
While this doesnt achieve centered rotor to bracket spacing I dont believe its an issue; neither side has a clearance larger than the pads backing plate.

I went ahead with cutting up the Miata rotor for a spacer.
I ran into some issues trying to get the rotor centric to the hub. This was obvious by a long lopey grinding noise when braking when the rotors were grinding into the caliper.
I was able to get this closer by checking rotor runout and adjusting with a dead blow hammer before tightening the lugs.
Initially I thought this could be solved by replacing the hub (and bearing) with the EX hub. Finding the EX hub proved to be very difficult. Hubs I ordered turned out to be the same as the Si hubs on the car.
A friend of mine turned up some spacers to center everything. These run from the hub face to proud of the rotor. Not an issue as my wheels center bore is larger than 64.1mm.
Between the milling down the caliper bracket or making a spacer and a centric ring this isnt exactly a bolt on affair. Hopefully the results are worth the effort!
