Default Chipped P28 runs fine, then solid cel

Has anyone ever ran into a condition where if you have a tune on a chipped ecu and you drive it and it drives fine, the next time you try to drive it the cel lights and car runs very rich? Does this sound like a bad tune, bad chip itself or a bad ecu?

When it runs rich, is the cel light on and can you rev past 4k?
