This is what I heard. Maybe something else is in the way.

Yes, which is why you need to connect the two grounds.

Back from the dead.
This write-up was excellent. Thank-you.
I modified your directions slightly in the following way:
As you know, some JDM 92-95 Civics came stock with a maplight assembly. I believe these were exclusively the models with a sunroof, but I am not 100% sure. <- EDIT: Incorrect. Models with and without sunroof in combination with a map light exist, though the style of map light is different - large (oversize) one w/sunroof, narrow (small) one w/o.
Anyway, the part number on these JDM Civics was 34256-SR3-901ZB; this is the exact same part number used on the 2-dr CDM 98-00 Si / 99-00 SiR, 4-dr CDM 98-00 Acura EL, 2-dr USDM 98-00 EX / 99-00 Si and 4-dr USDM 98-00 EX. (Note: All vehicles with sunroofs.)
I wanted to stay as true to the original Civic design as possible, so I obtained this part from an Auto Salvage yard, including a very long pigtail. The donor vehicle was a 2-dr USDM 98-00 EX. Cost: $20 (a bit high, IMO)
After cutting the hole as instructed, I fed the long pigtail down to the A-pillar and connected it to the dome light WHT/RED wire with a T-Taps. I crimped terminal O-rings to the end of a 14ga BLK wire and bolted these in through the original mounting/grounding holes on the map light assembly with a small screw and nut. This was grounded with another terminal O-ring not at the sun visor mount like you did, but at the sun visor clip which is much closer. I also added a small washer between the O-ring and the headliner to make the connection more reliable.
I am extremely impressed with the amount of light cast by the unit and think this is a pretty useful modification.
