Default Carbon Syncros

Anyone have this problem I Installed a final drive and a new set of carbon synchros about 5 months ago even a new CC clutch on a cable tranny S1 on the last 3 months it was getting harder to get 1st gear or second even with the car not running so i pulled the tranny last night and this is what a found only first and second synchro.

This is caused from the clutch not fully disengaging. I see it often with a twin disc clutch. It could be an adjustment issue or possibly a warped clutch disc.
