Default need someone who has a obd1 swap in a EF

I am having a problem with my O2 sensor
Well I downloaded the wiring instructions off of hybrid.com to go from 1 wire to 4 wire. Well while consulting my chiltons manual I found that the sensor should output .1 to .9 V. Well mine outputs 4.1-4.9 V. Now when I tested the sensor output I used chassis ground as reference.
Now heres the question...
Is my sensor bad? Or is it the input to the sensor incorrect?
I am not sure if a bad sensor can even read voltage that high?
Also would it have made a difference if I used the sensor ground wire as ground reference?
Just looking for any help I can get, this is the only code I have and I am getting sick of the CEL...
