1-4 wire o2 still with CEL 41

1-4 wire o2 still with CEL 41
Did the wire swap in the car, reset the ecu and the second it starts back up im getting a code 41. I did the http://www.hybridautomotive.com/tech/o2/#seven swap.
Green wire on o2 harness to the ECT green
White in the o2 harness to the old o2 sensor
spliced the orange/blk into AG (orange/blk) on the ecu
Yellow/black on the harness to the yellow/black on the Evap purge valve.
Swapped in another o2 sensor and its the same deal...any ideas?

double check the wires on the other side of the harness (on the O2 sensor)
green = green/white
white = white
black = yellow/black
black = orange/black
if that checks out take a meter and check that the orange/black wire connects to A6 of the ECU, and that the yellow/black has 12Volts on it when the ignition is on.
if those check out, check the ohms between the two black wires on the O2. it should be between 10-40 ohms.
green = green/white
