Default Clutch Disc Splines Stripped??

I got an Action 4 puck clutch with a brand new Fidanza lightweight flywheel. My car is a B20 swap with a B16 hydro trans and a Hasport conversion bracket. I had the install done at a local performance shop and the clutch and flywheel worked beautifully. I had upgraded the pedal assembly with a Hush Performance bracket to reinforce it but still used the cable setup. It was stiff, but not terrible....the cable was adjusted so the pedal take-up was about midway, not right off the floor. Driving very much non-aggressively one night about 2 months after install I just suddenly lost gears...terrible grinding noise. Still had pedal feel but it wouldn't go into any gears. Took the car back to the shop and they pulled the disc out and the splines were stripped smooth. TO bearing appeared to be fine, friction material was still fine.

The shop said that my Hasport conversion bracket was the issue... They said that it was causing the clutch not to disengage completely and gradually eat the splines of the clutch. I think that with the cable adjusted properly there shouldn't be an issue. A partially engaged clutch will cause the car to creep in gear and make it hard to shift. My car shifted beautifully and never crept at the light. I kinda think the TO bearing either slipped off or otherwise there was an installation issue.

The other possibility is that the wrong clutch disc was installed...but from I understand is that I have the larger spline trans and the cable B series disc won't fit.

Any insight would be very helpful!

I can only think of an incorrect spline being the cause.

Kinda what I thought too, not what other spline would fit but loosely on a hydro b16 trans?

What's the code on your transmission? Did the disc seem to fit loosely on the input shaft when you installed it?

I don't know the code on the trans....the disc did fit snug. Do they make a small spline B16 hydro trans?

Not that I know of, but would be interested to check the code and verify what it is. Should be on a sticker on the top/front of the case?
