it does sound like a clutch issue. need more details, like miles, how many clutch replacements, how do you drive?

If this is a manual transmission, it could be the clutch master cylinder, clutch slave cylinder, or the clutch disc itself. Your description is too vague to determine exactly which one.
Typically a hydraulics problem will cause hard shifts and grinds, so you probably just have a worn out clutch.
