Stalling & Hesitation at WOT...?

Stalling & Hesitation at WOT...?
thought i had all the gremlins taken care of....
guess not.
my tach is back to acting crazy... goes up and down, and doesn't sound to be matching the actual RPMs of the car.
the car generally lacks some power and hesitates...
and when i floor it, the car feels like it's going to stall, and the RPMs drop.
if i give the car too much gas, it feels like it's going to die on me...
on top of that a puff of white smoke came out of my tailpipe, but only for about 4 seconds, and hasn't done it since....
any ideas??

my guesses just from your post:
bad ground somewhere
poor fuel mngment
piston rings may me shot
cracked head gasket
