Default Slamming out of gear ?

I just bought this car, trying to get her ready for my state inspection. Finally got her running with no cel's or irractic idle. When I took her on a test-drive, i put her in 1st gear, letting it out the clutch (being easy on her) the front end like lifts up and slams down, while it does that i can feel the shifter wanting to push out of gear. it will not do it if i ride the clutch out. the mounts aren't brand new, i have seen worse. The Rear T-braket had loose nuts, however i tightened them up and still doing the same thing. the clutch is very soft even after bleeding it twice. so im guessing there isn't much a clutch left on her. if anyone has expeirenced this slaming out of gear issue or have a suggestion, feel free to let me know.
Car Info: 1997 honda civic hatch, d15b7 motor, d16y8 transmission, converted to manual.
recently did a iac 3-wire, and changed axles
Also my fans arent kicking on but the cluster doesn't say im over-heating, i did loop the ect ? plug and still got nothing, also switched relay's and got nothing. any help on that would be helpful also
