O2 sensor problem...

O2 sensor problem...
Here's the story...
When I first boosted my '99 EX w/ d16y8, I had to extend the primary o2 sensor wire from the back side of the motor to the front side. In doing so, some how the wiring got snagged by the axle and messed up the pigtail on the harness side. I took the harness of and took it to my buddy to have him fix the wiring and put a new pigtail on it.
Now, I plug the harness into my brand new o2 sensor and I'm getting a p0142 high voltage code for sensor 1 bank 1....I swapped the two ground wires on the sensor around and now I'm getting a p0131 low voltage code for sensor 1 bank 1....
What to do now?!?!?!?!?!

The harness should have a Blk/Yel (switched +12v for heater), Blk/Wht (heater control), Grn/Blk (O2 ground) and Wht (primary O2 signal) or Wht/Red (secondary O2 signal).
Get an O2 sensor that already has the connector attached.
