My findings after Pulling the clutch
Well after about 3 weeks of no Civic, I got around to pulling the tranny and clutch. I had expected to a see a spring popped out of the disk, but when I pulled the pressure plate back I heard and saw multiple pieces of metal from the stock B16 disc fall out . Well the flywheel is getting resurfaced and it and the new CM's should be going in this evening.
Here is the disk and few pieces that fell out with it.
Here's a closer look

Hey that looks familar. The very same thing happened on my old CRX. The spring retaining part of the disk broke off and would get wedged in the pressure plate. I didn't have any loose flywheel bolts though.
My symptoms were:
Sometimes you could not get out of gear/in gear easily....sometimes you could. The reson was that the loose piece of the disk would intermitantly(sp?) get caught in the pressure plate. This would only allow half of the pressure plate surface to lift away from the disk surface. The other half would still have semi-pressure on it. So basically I wasn't getting a full release of the disk.

Re: My findings after Pulling the clutch (Slow *** EK)
my buddies just changed a clutch on an SI a month or so back. Same part fell out of the disc.
his symptoms were a whistling/grinding noise when you push in the clutch and random grind noises and sometimes hard to get in gear.
Flywheel bolts werent loose either.

Flywheels bolts did NOT back out. They were fine.
Here are the symptoms:
A couple days before, I heard a slight whine comng from the engine bay when I shifted or sometimes at idle. I couldn't figure it out until things went bad. That night I was driving and shifting at a pretty high RPM and going into 3rd there was a very loud grind and it wouldn't catch. Then once I came to a stop and tried to put the car in 1st, the whole car shuddered and I had to rev to about 2-3k just to get going. Then pedal pressure was very low and it was extremely rough to get into gear.
I think that this may have been building up for some time (or deteriorating I mean) sometimes the gear would not engage and it would be difficult to just go in to gear when shifting and there was a slight grind in 3rd at high RPM. I'm putting in the new clutch tonight, so by tomorrow I should be able to tell if the trouble going in to gear was in fact the clutch or my shift linkage not being correct.
