Default Mini Me Problem

O boy, i spent all Saterday and Sunday to finish a mini-me swap on a eg dx hatch. a d16z6 head on a d15b7 block. after everything is in I had a extra plug left. i forgot here i took it out from. It hangs on the driver side a little bit below the intake manifold. it is wrapped in the black OEM plastic cover along with a similar plug that i plugged into the intake manifold. I couldnt find the right plug for it (actaully i can't find a open plug at all). So im wondering if there is suppose to be an extra plug? i believe its whiteish in color but im not sure, and ill will check and post it asap when i get home tomorow. I didnt have t vtec ecu so i am running it with my stock ecu without the vtec solinoid pluged up. So basiccly no vtec, just trying to start it. So with everything in, we tried to start it. It was being jumped because my battery was dead. It struggled at first then it started. then it revs up to 1500 rpm, then up slowly.. and slowly. it got to about 3000 and i got scared so i turned off the engine. then i tried it again, then it did the same thing this time i revved up a little bit faster. then when it hits 3500, it drops to 1500, then up to 3500 then again and again. i beleive this is due to my valve cover bolt, i was missing 2 so i plug one but i didnt plug the other because i didnt have anything to plug it with. I believe it might be because of the vaccum from that bolt hole? but i left it on for about 3 minutes, and a lot of smoke was coming out. not much from the tail pipe, but there was a lot of smoke from the valve cover bolt hole that was unplug and it seemed lek it was coming out from the headers. And my engine was throwing codes, the engine light was on.

These are the syptoms:

-revs to 1500, then 3500, then 250, then 3500 repeat
-check engine light on.
-smoke from valve cover bolt hole
-smoke from exhaust manifold area.

and these are the conditions on my car:
-a valve cover bolt is missing
-stock p06 ecu
-extra plug, i dont know where to plug it, will check asap
-timing might be off, i didnt have a timing light
-using old exhaust manifold because i couldnt afford to buy a new one, but looks ok.

What could be wrong?

What code are you throwing? It could be ICV.
