Default Limp mode code 16

I have a 89 DX hatch, Wired in obd1 ecu and did singe cam swap. Every thing seems to work fine at idle no codes, but when i go drive my car it will let me rev up to 5500 + maybe one or two times before it go's into limp and wont rev past 3200. I tryed saturated injectors and peak and hold with resistor box.. My injectors are wired ( B1 / A24 blk/yel for voltage, A1 A2 A3 A4 to injectors) Right now i have saturated injectors in they ohm around 10-12. Now.. I did a K/O test i get 12v to all blk/yel wires and signal ( less than 2v from ecu. sometimes my car wont start after i shut it off so it made me think the relay was bad... I tested the relay and i added more soder to the board. still dose the limp mode and randomly wants to start.. The code 16 only comes on when i make the injectors work as in rev the motor or try to drive it. Could this be the wrong type of relay? The car was DPFI before i did all this so im thinking i have the wrong relay?

It shouldn't be the relay. Borrow a fuel pressure test kit from a parts store and check your fuel pressure with and without the regulator vacuum hose attached.

Checked the fuel pressure and return line.. seems normal... Checked my wiring 10 times over again its getting the correct voltage.. If it was a fuel problem the car would stall and it never stalls. It runs rich.. i dont no if thats limp mode doing it or something else.

If the fuel pressure is too low, in some cases the pressure still provides enough fuel to support low RPM running but not enough to supply the injectors adequately at high RPM. It's a moot point if the pressure is fine, though.

It could be that there is a loose connection that is aggrevated by vibration like when starting the car, high RPM driving, etc. When it's not currently in limp mode, you could stop the car on a steep hill, facing upward, and try to gun it from a stop up the hill. If there is a loose connection affected by vibration, the engine torqing up a hill from a stop should put you right back into limp mode again.

No hills where i live only swamp and rednecks, But whats unusual is the car has the power it should from 0-6k rpm sometimes i can clear rpm band 2 times sometimes it will throw code 16 and limp mode before i pull out of my drive way. I did some more testing and i put the yel/blk wire to the 3 slot on main relay, and i ran the car till it went limp than rev to 3k with a voltage meeter to the yel/blk.. it ran 12-14v and never went lower than 12. did the same on signal wire from ecu they dont drop voltage. Still waiting on a fuel pressure test kit i keep leaving it at my shop.. its the only thing i have not tested as stupid as it sounds but its my last option here lol.

Did a fuel test, also put a adjustable fpr on and set it to 35psi. Im not loosing any pressure.

You'll get that code anytime an injector wire is disconnected or if its signal isn't making it through the circuit for some reason. You could borrow a noid light set from a parts store and watch how regularly each injector fires. If one misses certain pulses then that injector's wires have a bad connection somewhere. It wouldn't hurt to check any of the FI wires, connectors or their ground connections.
