prolly 200-300 at a shop
if you do it yourself, all you really have to do is grind it down really good, assuming it is only surface, then bondo to fill the missing spot and paint

depends on how deep the rust goes, and How good you are at painting, Could take a couple of hours could take a LOT longer... and end up costing you even more when you give up and go to a shop.
Have you ever painted anything before?

Most paint shops will do a touch up for >$100.
As for the rust. You said it was just surface, so like someone said, Grind it down/off. Bondo it up, you need to have some type of skill if it's a big area. You can't just slap a glob of bonda on and expect it to look good.
Actually it's more of the sanding part you need to know how to do, a regular sanding block is going to take longer but may yield better results. I can use an electric shaker sander and get it looking new.

you can do as mentioned with bondo, but if you have some friends or connections to help you can do as i recently did in my shop class. Had a 2" long piece of straight rust on the back quarter. I grinded it down, welded it up, bondoed, then touched it up. Not too hard if you some people to help. I think we charged around
$300.
$300.
