Did you make sure the washer was on the outside the cog gear?

the cam gear doesn't have a washer, only the crank. as to the slipping how is the belt tension?
id assume you just don't have enough tension on the belt and it ripped the belt over a tooth...
with enough tension on the belt it shouldn't slide off the cam gear either.

I know the cam gear doesn't have a washer, but if you don't have it on the crank it will ride off onto the balancer and you'll see it off at the cam.

prolly because u didnt tension the belt right when u put it all on and with the tensioner lose u take a socket and rotate the crank 3 teeth to put tension on the belt then tighten the tensioner pulley.

Hopefully you avoided engine damage after the belt slippage.
