Could be a few things. First is to check the ECU for codes. That's probably why it's cutting out. From what I hear, V-TEC won't even come on if you have a check engine light or ECU code. I have a 90 civic hatch with MPFI PM6 ECU and I had a coolant temp sensor wire that had a cut in it by the plug (so basicly the ECU didn't read it) and it would rev limit at 6500 RPM. As soon as I fixed it, it went to 7000 RPM. So, first thing I would do is check the codes. More than likely one of the sensors and/or wires that need to be fixed.

See your RPM when you're going in 3rd gear at 45mph.
Give us 1 or 2 instances of exactly what your tach is reading at a certain MPH in a certain gear.
We can then calculate what your RPM SHOULD be, and see if your tach is off.
Or you can plug a new tach in there - but I feel the former is more accurate.
