Crank pully fell off what would cause this.

Crank pully fell off what would cause this.
Started the car up this morning sounded kind of funny. Then once I pulled up to work, my crank pulley came flying off. So now i need a new woodruff key, and another new alternator belt phuq!... What would cause this...
next time should i use loctite, or teflon tape.
