Do a compression test. If you have the means of performing a leakdown test, that would help too.

What does the car smell like? Fuel? Antifreeze?

What color is the smoke:
White = coolant: most likely a blown head gasket, could also be an intake manifold leak. Either way, it needs to be corrected immediately.
Bluish = oil: could be a number of things such as piston rings, valve guides, etc. Not as bad, but should still be diagnosed.
Black = fuel: you're running incredibly rich. This isn't an immediate cause for alarm, but will destroy your o2 sensors and cat (not to mention gas mileage and performance) if you ignore it.

Any of these can cause the car to run like ****.
