I believe but am not sure that you need a manual ECU as well.

its a real pain...you need the manual ecu, the linkage, all new pedals, mounts, cut into the floor base, manual tranny etc. i would only do it with a complete swap. its pointless for a d series motor.

It might be just indented and have to punch it out.

I did it on my 93 ex. You would need to get the following parts:
Master cylinder
Slave cylinder
brake pedal
clutch pedal
clutch resorvoir
manual tranny
shift linkage
shifter
shift boot
clutch
flywheel
flywheel cover
flywheel cover bolts
manual Ecu
clutch lines
manual trans. Mounts
The best thing for you to do is go to the dealer and ask them for a print out of the following diagrams:
-shift linkage
-mastercylinder/slave cylinder...this one should show you all the lines and misc. bolts that you would need.
-pedal assy. You will need alot of bolts and brackets and screws that the diagram will show.
I think thats about it. Good luck
