Default parts list you need for a 5-lug conversion

Ok this is it, I've been researching, reading, investigating, studying and examining to get make clear how to do a 5-lug conversion, and I mean how you'll get all you need with no doubts or having to go back and find more parts. Now looking at the price you'd end up with if you were to order new parts, I do not recommend getting new parts, 1st because there are many scrapped ITRs (Integra TypeR), 2nd looking at the price list makes me feel like I've been slapped with a hefty fine!
I am basically putting the list of parts together, big or small, although I might need some of your input to wrap it up for all to see. Links to sites who'll sell and ship to you these parts are inclusive, so then you can either take it to the mechanic or even DIY.

All parts you need are from Integra TypeR 97'-98'

Below are the sites you who'll sell you the parts you need, followed by part #s. Note that on some pages you'll not need all the parts, so my goal is to clear this out. Here I need some input to clarify which part numbers are required and which are not required for the conversion. Below are which I am sure of, please (if you are sure) advice which to add to or remove from the list of REQUIRED FOR CONVERSION and I'll modify accordingly:

<U>BRAKE LINES (ABS)</U> this is from a 92-93 Integra, from here I believe you only need the Porportioning Valve 4040, part# 2, which should be same as the Civic Si that you need
